   Appointments
      
We try to see all patients by appointment and request that you call in advance so that we can reserve sufficient time for your visit. You have the opportunity to see us the same day you are sick by calling at 8:00am.  These appointments can not be scheduled prior to the day of the call and are scheduled on a first call basis.  We request a 24 hour notice for cancellations of scheduled appointments so we may make those appointments available to other patients.  Cancellations made with less than 24 hours notice may result in a $35 charge.


   Emergencies

A practitioner will always be available for emergencies 24 hours a day, 7 days a week. 

During office hours:  call the office, identify yourself and describe the emergency to the receptionist

After office hours:  call the office and follow the instructions on the automated message to contact the practitioner on call

 
 Telephone Calls

We encourage you to call with questions regarding any medical problem.  Our front office staff will relay information to the practitioners who will contact you as soon as possible, usually the same day.


   Lab Results


It is our policy to inform you of all laboratory results either by phone or mail.  If you do not hear from us within a week, please call ext. 1032.


 

Prescription
Refills



When calling for prescription refills, please have the container handy so that you can provide the needed information.  We request 24 to 48 hours notice for refills to be processed.
Refill requests can be made through ext. 1032.
   Payments

Payment, either in full, or the portion required by your insurance, is expected at the time of your visit.  We accept cash, checks, Visa, MasterCard, American Express and Discover.  There is a $30 charge for returned checks due to nonsufficient funds.

   Insurances

To meet your health insurance needs, we participate in a wide variety of insurances.  Please verify with your insurance company that we are in your insurance network.  We are happy to assist you with your insurance claims.  Our billing dept. can be reached at ext. 1011.

  Referrals

Please allow 3 business days for processing of referrals.  These requests can be made by phone, in writing or from your primary care physician. We will need your date of birth, insurance information, the name of the referring doctor, the name of the doctor to whom you are being referred and the date of your appointment if one has already been scheduled. Our referral dept. can be reached at ext. 1019.
