Patient Centered Medical Home Responsibilities



What is a Medical Home?

The Patient Centered Medical Home is an innovative, team-based approach to providing healthcare. A partnership develops between the patient, his or her primary care physician and a health care team. Together, following evidence-based guidelines for medical care, the team will coordinate the services you need to provide the best healthcare possible.

What can you anticipate as a patient?

Team Based Care:

Your primary care physician will work with the rest of the DWFP team of primary care providers, nurses and support staff. This team will work with you to provide all of your healthcare needs, no matter your age or stage in life.

Improved Healthcare Access:

We’ll make it convenient for you to get access to your healthcare.  Our practice offers same-day sick appointments, after-hours phone access and extended hours in the evenings and Saturdays.

Improved Communication and Access to Information:

You can communicate with a healthcare team member anytime during normal working hours by calling the office at 215-348-4914.

After-hours, for urgent matters, you can call our office and leave a message for the on-call provider.

Patient Responsibilities

To allow us to coordinate your healthcare needs, you will need to make a list of other healthcare providers you have visited with their contact information and reason for the visit as well as any recent test results that we may not have access to.  As the center of your team, and its most important member, you are a full partner when it comes to your healthcare.  It is your responsibility to supply the members of your team with the information they need to do an optimal job.
